Independent Driver / Truck Owner Application Form

The primary application capturing basic operator details and business background.

Who completes it: Independent owner-drivers.

Required for: All applicants.

Driver Code of Conduct and Platform Rules Acknowledgment

Formal commitment to CargoLink's professional service standards and safety guidelines.

Who completes it: Every driver accessing the platform.

Required for: All applicants.

Driver Operating Profile, Licence Restrictions, and Experience Disclosure

Comprehensive disclosure of licence class, driving history, and specific Experience across Grenada's parishes.

Who completes it: Every driver.

Required for: Mandatory for all drivers.

Vehicle Owner Authorization and Designated Driver Form

Authorizes a specific driver to operate a vehicle owned by another individual on the platform.

Who completes it: Vehicle owner and designated driver.

Required for: Cases where owner and driver are different people.

POD and Evidence Standards Acknowledgment

Confirms understanding of digital Proof of Delivery requirements using landmarks and photographic evidence.

Who completes it: The active driver.

Required for: All applicants.

Vehicle Capacity, Insurance, and Payout Verification Addendum

Final verification of cargo weight limits, insurance validity, and confirmation of payout structure alignment.

Who completes it: Vehicle owner.

Required for: All applicants.

Payout Beneficiary and Revenue Authorization Form

Identifies and authorizes the bank details or beneficiary for platform revenue payouts.

Who completes it: The legal owner or authorized financial lead.

Contractor Agreement Data Sheet / Schedule Intake

Captures specific data for contractual schedules and formalizes operational hours and parish availability.

Who completes it: The primary contractor/owner.

Scenario 1: Owner-driver

You own the truck and will be the only person driving it on the CargoLink platform.

  • Forms Required: A, D, E, F, G, H
  • Notes: This is the simplest onboarding path; all identification and payout documents must match your personal ID.
Scenario 2: Owner and driver are different people

You are onboarding a truck that you own, but it will be operated by an approved designated driver.

  • Forms Required: A, B, D, E, F, G, H
  • Notes: Form B is critical to ensure the owner authorizes the driver to operate on the platform.
Scenario 3: Payout goes to a different approved beneficiary

The revenue generated by the independent operator needs to be directed to a non-standard account or secondary beneficiary.

  • Forms Required: Core set + Form C
  • Notes: This requires extra verification of the beneficiary bank details before activation.
Scenario 4: Special licence or operating conditions

Operators with specific licence restrictions, non-standard availability, or unique risk history disclosures.

  • Forms Required: Core set + Form G detailed operating profile
  • Notes: Direct review by CargoLink support is usually required for these applications.

Which form is used if the truck owner and driver are different?

Use the Vehicle Owner Authorization and Designated Driver Form so that the owner can authorize the driver and confirm how the vehicle will be used on the CargoLink platform.

What form confirms who receives payout?

The Payout Beneficiary and Revenue Authorization Form is used to confirm who is authorized to receive payouts and how those payouts should be directed.
